    Therapeutic & mechanically altered diet orders
    I am reviewing a menu for a LTC facility. There is a new "high nutrient" diet that provides a few fortified foods on a regular diet. This menu also has a mechanical soft and pureed diet which are close to the regular diet, but does have some substitutions when the food item cannot be appropriately altered mechanically . I'm concerned that if this high nutrient diet is ordered for a resident on a mechanical soft or pureed diet, they may be given the regular texture. Should I expect all staff will be trained to understand the fortified food goes with what ever texture the resident is ordered? Anyone else in LTC have a similar situation, and how do you deal with it.
