REDUCING THE BURDEN OF DIABETES: NORTH CAROLINA'S GUIDE TO DIABETES PREVENTION AND MANAGEMENT
During this session you will learn how the North Carolina Diabetes Advisory Committee came together to develop a Diabetes Prevention Plan while considering how that plan should and will tie into the Management Plan. You'll leave with knowledge of strategies that can be implemented not only in NC where this plan was created but in communities around the US. We'll talk about strategies that can implemented on the community level, what workplaces can do as well as what individuals can do to prevent or manage diabetes. These are practical strategies that can be implemented for success. We'll also review how implementing these strategies can be measured. This can be helpful for grant writing purposes.
Upon successful completion of this one-hour course, the participant should be able to:
  • Distinguish strategies for implementing this Diabetes Action Plan in your community, workplace or professional role.

  • Identify strategies for health care providers that will assist in integrating and implementing this Diabetes Action guide within the community and workplace.

  • Discuss the role of DSME/S in reducing the burden of diabetes related to cost, efficacy and identify the four critical points along the diabetes continuum where DSME/S should be initiated.
